What would you like to tell us about your practice?

The APF’s development has been supported primarily by the MGH, the MGPO (Mass General Physicians Organization), the Stoeckle Center for Primary Care Innovation, and the Center for the Integration of Medicine and Innovative Technology (CIMIT). The APF care model has been designed with the notion that the patient is at the center of a collaborative, interdisciplinary care team. This team is keenly aware of the patient’s health goals and priorities, and strives to ensure that those goals are met by: -Providing a culture of service and collaboration -Engaging the patient both during visits to the practice and in between visit to participate in his/her care and -Meeting the patients needs at any time and in any location.
